Clients who participate in Service Navigation will work collaboratively with their Service Navigator to:
Complete a comprehensive and validated survey assessment
Identify clients’ service needs
Identify clients’ strengths
Create an individualized service plan
Execute the service plan via in-house support and referrals to partner organizations
Our Service Navigators have intimate knowledge of the South San Diego community and are adept at modeling soft skills, engaging in warm hand-offs, and following-up with clients to ensure referral uptake.
The goal of this aspect of the program is to improve access to and use of health and social services among young adult reentrants.
