Who is eligible to participate in UCSD RELINK?
To participate in our program, you must be:
  • Age 18-26
  • On probation in San Diego County (not AB109)
  • Living in the South San Diego region at the time of referral (anywhere south of the SR94 freeway)

How much does it cost to participate in UCSD RELINK?
  • This program is free for clients, partners and probation.

​How do you receive referrals?
  • Referrals are accepted from:
    • San Diego County Probation Department via the CRD
    • UCSD RELINK Community Partners by phone or our Online Referral Form.
    • Eligible individuals (self-referral) by phone or our Online Referral Form.

Where are UCSD RELINK services provided?
  • We operate out of the District Attorney’s county-funded Community Action Resource Engagement  (CARE) Center (12 N. Euclid Ave National City, CA 91950) in National City. See our Location page for a map.

What can a client expect from the UCSD RELINK Program?
  • An outreach call from one of our Service Navigators.
  • An appointment at the CARE Center.
  • Completion of a Welcome Survey that helps our team to understand where the client is in their life and what support they need.
  • The choice to participate in Service Navigation only (to improve access to and use of health and social resources/services) or Service Navigation and a comprehensive and personalized Health Fellowship Curriculum (to prepare the client to become an independent adult by harnessing their innate strengths and endowing them with new skills).​

What if I want to return to the UCSD RELINK program?
  • Our doors are always open to current or former clients - if you've lost touch with us and want to reach out, please contact us!
